Set 27 years after the events of the second film (I Still Know What You Did Last Summer, 1998), this film tells the story of a group of teenagers, Danica, Eva, Tyler, Max, and Jonah, who try to cover up a car accident that killed a pedestrian.

As time passes, they begin to be terrorized by a mysterious killer who seems to know their dark secret. As the terror intensifies, they realize that similar events have happened before.

They realize the stalker is imitating the style of the legendary serial killer who terrorized Southport in 1997. Driven by the constant threats, they eventually seek help from survivors of the past tragedy. They turn to Julie James and Ray Bronson, two survivors of the massacre.
